The culprit is Nakanishi Kei. She did it to his boyfriend who died three years ago. When he suddenly got into an accident, what her makes it strange is the car parked next to an ambulance, her boyfriend was Ogawa Katsuyuki who died before reaching into the hospital. While Kei was investigates from the owner's car. It turned out that Remi's car who was parked. When she asks her about it, she laughly said at her saying "He was just never meant to live long in the first place, idiot". Also, she's the one who put the fake will beside her. Remi used fake names to hide her debt from shady business. She disguised as Kirisaki Remi to commit her perfect crime. Mostly, the police thought it was a suicide, of course the fake Remi said to the landlady that she wanted to die. She surrenders herself to Kogoro and Conan.
